Health Risk Assessment (HRA)

The health risks and conditions among your organization's workforce can be analyzed through OHM using the RiskCalc™ health risk assessment (HRA), a full-featured Internet-based risk evaluation tool that can help you to identify risk management opportunities, and promote awareness and healthy behavior change.

The HRA is designed to evaluate a variety of behavioral and biomedical health risks and other health-related information, including:

weight/BMI	alcohol use
body fat	stress
blood pressure	dental care
cholesterol	sun exposure
triglycerides	safety
blood glucose	allergies
exercise	personal and family medical history
nutrition	current medications

Personal Health Risk Report

Health Risk Assessment Graph #1 Upon completing the RiskCalc™ HRA, the participants will receive at the website a personal health risk report, which they can view and print out at their convenience.

The attractively formatted, reader-friendly report contains current health risk status information and healthy action plans. The risk reduction messaging provided in the report is individually tailored based on the data collected from HRA participant's stage of readiness to make positive changes in their health-related behaviors.

Physician Summary

A one-page summary of the HRA results will be generated for printing at the HRA website for the participant to share with their health care provider.

Health Risk Management Reports

Health Risk Assessment Graph #2 To assist your organization in addressing and managing the health risks of your employees, dependents, retirees, or others, the following health risk management reports are provided, based on the HRA participants:

Group Summary Report - This report provides the aggregate group results for the individuals in your organization who completed the HRA. Group results for each of the health risk areas assessed, as well as risk for specific health conditions, are provided in the report, using both text and helpful tables and charts.

Health Counselor's Report - This two-page personal health risk summary report, which is more detailed than the Physician Summary, is available for use in conducting follow-up health risk counseling. The report is conveniently formatted for reviewing the HRA results with the participant and setting personal management goals for identified health risks.
